Systems Failure is a role-playing game written by Bill Coffin and published by Palladium Books in 1999.

The premise for the setting is that the Y2K bugs were real, and invaded earth at the end of 1999 leaving a post-apocalyptic world in their wake.

The tone of the game mimicks Alien Horror/Splatter films like Tremors and Aliens, originally intended as a satire, the published game was more serious. Big guns, bigger bugs and some wacked out powers in a post-apocalyptic setting.

The thing about very structured systems is that people come to trust them too much, so failure can go unnoticed until it's too late.
